Caregiver Support:

Family members and friends are a vital part of a participant’s care team. CEI social workers, home care providers and other care team members work with family caregivers on a regular basis to make sure participants are safe and healthy in their living situations.

We provide many resources to caregivers:

·        Help with transportation of participants to and from medical appointments and the PACE day center

·        Safe and secure day care for their loved ones

·        Direct contact with CEI care team providers and representatives for questions and concerns about their loved one’s condition

·        Referrals to community services and caregiver support groups
