HOW DOES SOMEONE QUALIFY FOR CEI SERVICES?

In order to recieve PACE services through CEI, seniors must meet the following criteria:

• Age 55 and over
• Resident in CEI’s Service Area (Hayward thorough El Sobrante)
• Eligible for nursing facility level of care as certified by the State Department of Long Term Care 
• Able to live in a community setting without jeopardizing one’s health or safety.
