PRIMARY CARE PHYSICIAN (Full Time)
The Center for Elders Independence
is a non profit organization serving frail elders in the East Bay. Our innovative, all-inclusive,
long-term health care program (PACE Program) provides the frail elderly
with all the care they need, allowing them to maximize their health &
independence and remain close to their family members and within the
community. We currently have an excellent opportunity for a Full-Time Primary
Care Physician.
DUTIES:
(Primary duties and
responsibilities – may not include all of the duties assigned)
1.
Comprehensive
history & physical on protective enrollees.
2.
Participation
with IDT in development & implementation of Care Plans.
3.
Supervision
of mid-level practitioner(s).
4.
Support
on the CEI medical program.
5.
Evaluation
and treatment of patients with episodic illness, management of choric
illnesses.
6.
Care
of hospitalized participants.
7.
Management
of patients in nursing homes.
8.
Work
closely with multidisciplinary team.
QUALIFICATIONS:
1.
Must
hold an M.D degree and a current unrestricted license to practice medicine
in California.
2.
Formal
training in Internal Medicine or Family Practice.
3.
Desire
to work in the framework of a PACE model interdisciplinary team, and to
understand and perform the physician role within the team.
4.
A
current Drug Enforcement Agency Certificate.
5.
Experience
in working with a frail or elderly population.
6.
Good
communication, leadership and interpersonal skills.
