CEI is a Program of All-inclusive Care for the Elderly
(PACE), a long-term comprehensive health care program that enables frail older
adults to live at home in their communities for as long as
medically and socially feasible. CEI is committed to helping
frail older adults maintain their independence, dignity and
quality of life. CEI is a Health Plan which provides all medical
care (24-hours-a-day), a PACE Day Center, home care services and transportation.
PACE creatively coordinates the care of each participant
enrolled in the program based on his or her individual health
and social needs with the goal of enabling older individuals to
remain living safely in their community.
INTERDISCIPLINARY TEAM
Your CEI interdisciplinary team members are composed of a
primary care physician, PACE Center manager, registered
nurses, physical, occupational and recreational therapists,
social worker, dietitian, PACE Center health care workers, and
transportation representative.
As needed, your interdisciplinary team mobilizes other
services including medical specialists, laboratory and other
diagnostic tests, hospital and nursing home care. When you
first join CEI, you will receive a comprehensive in-depth
assessment from all of your CEI interdisciplinary team
members. These assessments will be made every six months, as
well as when there is a change of condition or a request for
services. You and your caregiver(s) will continue to
play an important role in managing your health and
independence.
Continuous monitoring and highly integrated acute and
long-term care services allow the CEI interdisciplinary team
to successfully manage the complex, constantly changing
medical, functional and psycho-social concerns faced by the
disabled elderly person.
Because the CEI interdisciplinary team members are in
frequent contact with you, they can detect even the most
subtle change in condition and modify your treatment plan. The
interdisciplinary team has complete flexibility in tailoring a
care plan for your needs.
You will get to know each of your interdisciplinary team
members very well, for they will work closely with you to help
you remain as healthy and independent as possible. Before you
receive or stop receiving any service from CEI your
interdisciplinary team must approve it. They will assess your
needs as often as is required.
