Eastmont PACE Center
CEI’s Eastmont PACE Center, located on the upper level of the Eastmont Mall complex in East Oakland, opened in Summer 2001. The PACE serves Alameda County residents of Hayward, East Oakland, San Leandro, Castro Valley, San Lorenzo, and neighboring communities.
Deborah Schwark, RD, Interim Center Director
Deborah Schwark has been with CEI since April 2008 after relocating to California from Wisconsin. She started as a contracted Registered Dietitian at Josie Barrow Center and Eastmont Center, both located in Eastmont Town Center. In late 2014, Deb joined the CEI team as an employee. She worked as the dietitian for both centers until late 2016, when she was promoted to Associate Center Director. Deb is currently working as interim center director for the Eastmont Center.
Dr. Qiratulanne (Annie) Khan, Primary Care Provider
Qiratulanne (Annie) Khan is a Bay Area native providing primary care at the Eastmont Center. She is Board Certified through the American Academy of Family Medicine and brings a wide range of clinical, research, and teaching experience to CEI’s PACE program. She is multilingual in Urdu, Punjabi, and Hindko and has worked in Pakistan, India, Ethiopia, Bolivia, and Navajo Nation in the U.S. In California, she has worked with various urban underserved populations in Stockton, Sacramento, and Berkeley. She has a special interest in vulnerable, underserved populations and is excited to be serving her East Bay community.
She received her doctorate through Touro University College of Osteopathic Medicine in California, completed a residency in Family Medicine at Boston Medical Center (Boston University) and a two year fellowship in Global Health through UCSF HEAL Initiative. She also earned an MPH at UC Berkeley and BS in Biological Sciences at UC Davis. She enjoys hiking, running, traveling and is working to fulfill her dreams of doing the Mt. Everest Base Camp Trek in Nepal in the next two years.
Kathi Ruel, Primary Care Provider
Kathi Ruel provides primary medical care at Eastmont Center. Before joining CEI in 2006, Kathi worked at Highland Hospital for 24 years, 13 years in Recovery Room and 11 years as manager of Employee Health. She was also a house supervisor at John George Psychiatric Pavilion and Fairmont Hospital. At CEI, she has served on the Policies and Procedures and Infection Control Committees, and helped introduce and implement the POLST form for advance care planning. She received her master’s degree from the University of California, San Francisco, and is board certified as an Adult Nurse Practitioner.
Jennifer Low, Primary Care Provider
Jennifer Low is a board-certified Adult-Gerontology Nurse Practitioner providing primary care at the Eastmont and San Pablo clinics since 2016. She earned her master’s degree in nursing at the University of California, San Francisco, and her bachelor of science degree in nursing at Columbia University. She spent three years working as a medical/surgical nurse at NYU Langone Medical Center and Mills-Peninsula Hospital. She belongs to the California Association of Nurse Practitioners and the American Association of Occupational Health Nurses.
