San Leandro PACE Center
CEI’s San Leandro PACE Center, which opened in Fall 2017, serves Alameda County seniors living in Hayward, East Oakland, San Leandro, Castro Valley, San Lorenzo, and neighboring communities. This state-of-the-art facility combines a PACE Clinic with an Adult Day Health Care Center.
Deborah Schwark, RD, Center Director
Deborah Schwark has been with CEI since April 2008 after relocating to California from Wisconsin. She started as a contracted Registered Dietitian at Josie Barrow Center and Eastmont Center, both located in Eastmont Town Center. In late 2014, Deb joined the CEI team as an employee. She worked as the dietitian for both centers until late 2016, when she was promoted to Associate Center Director. In 2018, Deb became Center Director at San Leandro.
Lincoln Sison, MD, Primary Care Provider
Dr. Lincoln Sison leads the inter-disciplinary team and provides medical care for participants at CEI’s San Leandro PACE Center. He has built a strong geriatric foundation working as a hospitalist and as a physician at various skilled nursing facilities in Baltimore, Maryland, for several years before moving to California in 2018. His clinical expertise and dedication to his patients was recognized when he was named Outstanding Physician of the Year (2018) by the Mid-Atlantic Medical Directors Association.
Dr. Sison has organized and participated in volunteer medical mission projects; mentored Geriatric Fellows at Johns Hopkins Medical School; worked with research teams in New York; and co-written journal articles. In addition to English, Dr. Sison is fluent in Tagalog, and speaks basic medical Spanish and basic Fookienese (Chinese dialect). He earned his medical degree at University of Santo Tomas in the Philippines in 2008 and finished his Internal Medicine training and subsequent Geriatric Medicine fellowship at Mount Sinai Health System hospitals in New York City in 2015. He is Board Certified in Internal Medicine.
Lisa Seng, MSN, AGPCNP
Lisa Seng is a Nurse Practitioner specializing in geriatric care. Prior to joining CEI’s staff in 2020, she worked at Penn Presbyterian Medical Center in Philadelphia and MedStar Georgetown University Hospital in Washington, D.C. She holds the Adult-Gerontology Primary Care Nurse Practitioner certification from the American Nurses Credentialing Center. As a research assistant, she conducted chart reviews on advanced care planning for home-bound seniors and co-authored an article on PTSD assessment and treatment for veterans. She earned her bachelor’s and master’s degrees in nursing from the University of Pennsylvania in Philadelphia. She has also held a teaching credential in English/Language Arts from UC Berkeley and has taught at an Oakland middle school.
