Kathleen Edmunds, MSHCA, BS, PT

Director of Home Care and Care Transitions

Kathleen Edmunds became Director of Home Care & Care Transitions in 2021, with oversight of CEI home care aides, RNs, LVNs, office staff; and the Care Coordination Service Center personnel.

She has had a diverse and wide ranging career in health care. She practiced physical therapy in hospitals, residential rehabilitation, private practice and community centers. She trained as a clinical researcher, and implemented the Clinical Research Program at Samuel Merritt University & Highland Hospital in Oakland for patients with diabetes.

After earning her Master of Healthcare Administration, she served as a project manager of the Racial and Ethnic Approach to Community Health grant for the San Francisco Department of Public Health. Kathleen valued her experience in extensive racial humility training while working in the community for the project.

Returning to physical therapy, Kathleen joined CEI as the Rehabilitation Manager in 2019, managing a department of over 30 physical, occupational, massage, acupuncture, and speech therapists, and rehab aides. She designed and implemented a new rehab departmental structure that increased collaboration, improved communication among facilities, increased accountability and standardized operations across the centers. In response to COVID restrictions, she guided the shift of services from a center-based model to a home/community-based model.

She earned her Bachelor of Science degree in physical therapy from Cleveland State University, Cleveland, Ohio, and her Master of Science in Health Care Administration from California State University, East Bay, in Hayward. In her spare time, she raised her son who is now in the US Air Force. Kathleen enjoys visiting with friends and family, is an avid Zumba enthusiast, and loves to travel.
