Home and Community Based Alternatives Waiver Program

The Home and Community Based Alternatives (HBCA) Waiver Program is a California State comprehensive care program that empowers at-risk and vulnerable participants to live at home. HBCA works with Medi-Cal to provide medical, behavioral, and in-home care services to you while living at home or in a community-based residence, so you don’t have to move to a nursing facility.


Center for Elders’ Independence is the administrator of the HCBA program for Alameda and Contra Costa Counties.

Care Coordination

CEI does not provide direct care, instead, we work with your insurance benefits by managing, directing, and coordinating services to help you maintain optimal health and safety from the comfort of your own residence. 

We identify and arrange medical and community services, such as personal care, nursing care, long-term services and local community support systems. The level of services HBCA participants receive may increase based on their individual needs.

No need to change doctors – we work with your current healthcare providers.

HBCA participants have the freedom to continue seeing their preferred doctors or begin receiving services from qualified doctors in their area that are included in their Medicare and Medi-Cal plans. They will not be restricted to HBCA-specific doctors, regardless of their participation in the program.

The HBCA waiver program offers additional care to keep participants at home. HBCA participants live in a community-based residence and receive services there, as an alternative to a skilled nursing facility or other long-term care setting. This residence can be privately owned, secured through a tenant-lease arrangement or be the residence of a participant’s family member.

The waiver program is for people of all ages. Currently, HCBA clients range in age from two months to 101 years old.

What is HCBA?

The Home and Community-Based Alternatives (HCBA) Waiver Program is a California State approved medical services program that allows significantly at-risk and vulnerable people to obtain medical, behavioral health, and in-home care services enabling them to live at home instead of moving to a nursing facility with their current Medi-Cal account.

Center for Elders’ Independence is the administrator of the HCBA program for Alameda and Contra Costa Counties.

Through this program, HCBA participants live in a community-based residence instead of a skilled nursing facility or other long-term care settings. This residence can be privately owned, secured through a tenant-lease arrangement or the residence of a participant's family member. 

How does HCBA work?

CEI has a dedicated team of Registered Nurse Case Managers, Patient Care Navigators and Social Workers who determine a person’s needs and then orders the necessary resources such as nursing care, IHSS personal care or home modifications.

Center for Elders’ Independence does not provide direct care; instead, CEI manages, directs and coordinates all of the various services for an individual person. 

As an HCBA participant, the level of services you can receive may increase based upon your individual needs. 

  • You keep your Medi-Cal doctor.
  • You keep your IHSS provider. 
  • The HCBA waiver offers additional care to keep you at home. 

How is HCBA different from PACE?

Both HCBA and PACE allow participants to receive care and remain independent outside of a nursing facility. However, PACE is only for seniors 55 years and older. 

Who qualifies for HCBA?

To qualify, you must currently be on Medi-Cal or apply for Medi-Cal. In addition, you must be eligible for or currently receiving care from a Medi-Cal funded nursing facility, subacute facility, acute hospital, or Intermediate Care Facility-Developmental Disabilities/Continuous Nursing (ICF-DD/CN).

Age is not a requirement to join the waiver program. Currently, HCBA participants range in age from two months to 101 years old. 

How do I apply?
