The Home and Community-Based Alternatives (HCBA) Waiver Program is a California State approved medical services program that allows significantly at-risk and vulnerable people to obtain medical, behavioral health, and in-home care services enabling them to live at home instead of moving to a nursing facility with their current Medi-Cal account.

The waiver program is for people of all ages. Currently, HCBA clients range in age from two months to 101 years old.

Center for Elders’ Independence is the administrator of the HCBA program for Alameda and Contra Costa Counties. CEI has a dedicated team of Registered Nurse Case Managers, Patient Care Navigators and Social Workers who determine a person’s needs and then orders the necessary resources such as nursing care, IHSS personal care or home modifications.

Center for Elders’ Independence does not provide direct care; instead, CEI manages, directs and coordinates all of the various services for an individual person.

Through this program, HCBA clients live in a community-based residence instead of a skilled nursing facility or other long-term care settings. This residence can be privately owned, secured through a tenant-lease arrangement or the residence of a client’s family member.

As an HCBA participant, the level of services you can receive may increase based upon your individual needs.

  • You keep your Medi-Cal doctor.
  • You keep your IHSS provider.
  • The HCBA waiver offers additional care to keep you at home.
