Eligibility Criteria

Are you or a loved one living with multiple health challenges, struggling with daily tasks, or managing several medications? CEI is here to help.

PACE Criteria

To join CEI PACE, you must:

  • Be 55 or older
  • Live in our service area
  • Be living with multiple health challenges
  • Be able to live safely in the community with the help of PACE
  • Meet the level of care requirements as determined by the California Department of Health Care Services

The CEI PACE enrollment team will conduct an evaluation for the need of nursing home level of care as part of the pre-enrollment assessment.

If you enroll in CEI PACE, you must receive your care from CEI contracted providers, or you may be responsible for the cost of care.

HCBA Criteria

To qualify for the HCBA, you must be a Medi-Cal recipient with high-level care needs. You'll receive personalized care management from a nurse and social worker to ensure you can safely stay at home or in your community.

About PACE

How much does PACE cost?

Cost is not a barrier to joining Center for Elders’ Independence. 

Medicare and Medi-Cal reimburse CEI for care. If you are enrolled in Medicare and Medi-Cal, you will pay nothing, or you will pay your Medi-Cal “share of cost.” 

CEI also accepts patients with Medicare or Medi-Cal alone, as well as those who wish to pay privately.

What happens when I apply to PACE?

You’ll have a home evaluation and a medical evaluation after you apply to CEI. 
We’ll also request your medical records, so we can demonstrate your needs to the State of California.

What happens when I apply to HCBA?

After you apply for the HCBA Waiver, a nurse and social worker will reach out to assess your needs. Based on these conversations, they'll create a plan outlining the care you require and who will provide it. Once the plan is complete, your doctor will sign it, and it will be submitted to the waiver agency for state approval.
