Philip Dah

Philip Dah
I’m a big believer in social justice, especially for the most vulnerable seniors with health issues. I lost my mother close to 20 years ago. She lived alone without much support and care through multiple health issues. The PACE model of care draws me to CEI. Seeing PACE in action reminds me of what could have been done for her and I am driven to improve the living condition of other seniors, who have sacrificed so much for us.
After graduating with my bachelor’s in business administration from the University of Ghana and moving to the United States, I began my career working with unhoused individuals and families. I come to Center for Elders’ Independence with close to 25 years experience managing homeless services including shelters, permanent supportive housing, drop-in centers, and food programs in the South Bay.
I came across many seniors who were homeless due to health and other reasons. I chose to join CEI as I’m passionate about serving elders.
I speak several Ghanaian languages including Fante, Twi, and GA in addition to English.
My family of my spouse and three children all enjoy playing soccer as well as camping at music festivals, cooking and traveling.

Eileen Kim

Eileen Kim
I respect our elders and see being a part of the PACE team as a way to help them stay healthy, mentally and physically. I like to encourage participants to care about all aspects of their life. Elders are still people that need treatment but might need more help getting it. I’m drawn to PACE because of the integrated teams and all around care beyond medication.
I started my physician journey at UC Berkeley in the pre-med program and then earning my medical degree at Ross University School of Medicine. I spent my early years as a physician in Merced, CA. I then joined the South of Market Health Center in San Francisco as a family physician, then promoted to Site Medical Director.
I was inspired to work with seniors and joined Center for Elders’ Independence.
I speak English, basic Korean, and am learning medical Spanish.
Outside of the clinic, I find yoga, traveling, cooking and enjoying the arts to be passions of mine. I like to focus on my family and friends and extend that out to the community. I try to remember every day to love life and to love those around me.

Dr. Rachel Fogel

Dr. Rachel Fogel
While in medical school, I decided to devote my career to older adults. The geriatricians I met in training, who emphasized whole-person care and worked to improve quality of life and function, inspired me. The patients inspired me too, with their lifetime of experiences and individuality. I enjoy a patient-centered approach to health and wellness, and enjoy partnering with people on their healthcare goals.
I trained at Johns Hopkins School of Medicine and completed my residency in Primary Care Internal Medicine at UCSF. Since then, I have worked as a geriatric hospitalist, primary care physician, and associate medical director at LifeLong Medical Care.
I left this satisfying career to participate in the team-based model of care at Center for Elders’ Independence, a unique gem in our healthcare system.
Outside of work, I spend a lot of time cheering on my son at Little League baseball games.
I enjoy sailing, the beach, reading, and classical music. I hail from Cleveland, Ohio, and have made my home in the San Francisco Bay Area since the late 1990s.

Kristi Elliott

Kristi Elliott
It’s now more important than ever to understand our collective history from all kinds of perspectives so we don’t make the same mistakes. I learn so much as I hear the stories our participants share with me. Being able to access medical care, mental health resources, physical therapy, dentistry and other specialties all in one place allows seniors a chance to relax and heal. As a provider, I see a patient thrive as all the pieces of a healthcare plan come together and work wonders.
After spending six years as a journalist for the Chicago Tribune, I shifted careers by attending nursing school at Truman College in Chicago earning an ADN degree in 2011. After three years as a registered nurse, I transferred to San Francisco location within the Kindred Healthcare Centers in 2013.
I joined Center for Elders’ Independence also as a registered nurse in 2015 and that’s when I fell in love with seniors. I decided to earn a Masters of Science in Nursing from Holy Names University in Oakland and become a nurse practitioner. I am very excited to return to CEI to continue my mission of making a positive difference in lives.
I speak English and limited Spanish. When I’m not working, I enjoy spending time with my fabulous husband and our growing collection of cats, dogs and turtles. We like to hike the beautiful, lush green trails in the Bay Area along with yoga and travel.

Jessica Medestomas

Jessica Medestomas
Preventing health complications and empowering others to take control of their health is inspiring to me every day. With PACE, I can get to know the seniors far better to help them achieve their health goals and what matters most to them.
My grandfather and our family was well-taken care of after experiencing a stroke by a multi-disciplinary team of experts. This inspired me to become a nurse and I’m excited to be a part of the PACE multi-disciplinary team here at Center for Elders’ Independence.
I studied nursing at Sonoma State University earning both my Bachelor’s and Master’s in Nursing there. After completing my studies, I started my career as a registered nurse in Napa Valley at a skilled nursing facility. From there, I grew my career to nurse practitioner in Sacramento. Before joining CEI, I worked as a nurse practitioner at a family care clinic serving a wide range of people.
I speak English. On weekends, I enjoy spending time with my family and three dogs along with traveling, visiting Disneyland, attending SF Giants games and trying new foods. I’ve lived in Northern California for most of my life.

Mary “Jenny” Kuang

Mary “Jenny” Kuang
I strongly believe that independence is an important aspect of healthcare, especially for older adults. I am inspired to provide compassionate care that enhances their quality of life. It is deeply fulfilling to me.
While working as a registered nurse in a skilled nursing facility, I saw how compassionate care could significantly improve the lives of my patients. I felt a strong calling to focus on senior healthcare so I could make a profound difference in senior lives.
Born and raised in the Philippines, I earned my masters in nursing at Concordia College. Shortly afterwards, I moved to the United States with my parents and siblings landing in the San Francisco Bay Area. I decided to further my education by attending and graduating from Samuel Merritt University, where I earned my Family Practitioner Master’s degree.
I speak fluent English and Tagalog.
During my free time, I love taking walks, watching movies, and having fun being a new parent to a baby girl. I enjoy spending time with my family.
