Michael Louder
Michael Louder
I possess a strong passion for promoting independence and advocacy among seniors. Being a part of their support system and serving as an advocate for older adults has provided me with immense joy and fulfillment. With over 20 years of experience in healthcare, I have consistently committed myself to serving underserved populations.
I earned my master's degree in business administration from California State University, Sacramento, where I was raised. My dedication to assisting others has taken me from Nevada to Arizona, and I have now come full circle returning to the Bay Area to work with Center for Elders’ Independence. My background in both business and healthcare enables me to cultivate a collaborative environment that prioritizes the well-being of our participants while ensuring the effective and efficient operation of the center.
I speak English.
In my leisure time, I enjoy traveling and savoring exquisite cuisine with my wife.
Dr. Rachel Fogel
Dr. Rachel Fogel
While in medical school, I decided to devote my career to older adults. The geriatricians I met in training, who emphasized whole-person care and worked to improve quality of life and function, inspired me. The patients inspired me too, with their lifetime of experiences and individuality. I enjoy a patient-centered approach to health and wellness, and enjoy partnering with people on their healthcare goals.
I trained at Johns Hopkins School of Medicine and completed my residency in Primary Care Internal Medicine at UCSF. Since then, I have worked as a geriatric hospitalist, primary care physician, and associate medical director at LifeLong Medical Care
I left this satisfying career to participate in the team-based model of care at Center for Elders’ Independence, a unique gem in our healthcare system.
Outside of work, I spend a lot of time cheering on my son at Little League baseball games.
I enjoy sailing, the beach, reading, and classical music. I hail from Cleveland, Ohio, and have made my home in the San Francisco Bay Area since the late 1990s.
Marie Adachi
Marie Adachi
As a geriatrician, I strongly believe that interdisciplinary collaboration is the foundation for good care, especially for vulnerable older adults. PACE is a healthcare model that fully embraces this concept, and I am inspired by all of the team members that come together every day to support the participants. I find it so rewarding to have the time to get to know each individual participant, and to help navigate their healthcare in a way that works for their unique situation, goals and values.
I earned my medical degree from The University of Chicago, Pritzer School of Medicine, and completed my internal medicine residency and my geriatric medicine fellowship at UCLA. I am now board certified in geriatrics and internal medicine. I joined Center for Elders’ Independence in August 2025.
I speak English and conversational Japanese.
Outside of the clinic, I love to travel to visit family and friends and explore new places. On a typical weekend, my spouse and I enjoy taking our toddler to the Oakland Zoo to see all the animals or taking our two dogs out to our local hiking trails. Doing jigsaw puzzles and reading are also favorite activities when I have some quiet time. I grew up in the Bay Area and have been living in the East Bay since 2020.

I really fell in love with caring for elders during the last year of my training, while working at a geriatric clinic in San Francisco's Tenderloin neighborhood. I was and continue to be amazed by my patient's stories and connection to history of place.
I received my Master of Science in Advanced Practice Nursing from UCSF in 2024. Afterwards, I completed a new graduate residency program at a nonprofit health clinic in the East Bay. I joined CEI in April 2025.
I am attracted to the PACE model as a health care provider because my patients can participate in their own care and remain integrated in the community and their homes. I love that all of my patients can enjoy nutritious meals, movement therapy, and social connectivity through the day centers and are deeply cared for by the interdisciplinary care teams.
I speak English and Spanish.
Before returning to school to become a nurse practitioner, I used to run a farm and growers' cooperative, so getting time outside continues to be crucial for me. I love to swim and dote over my backyard dahlias.

Josephine “Josie” Lee
My mother inspired me to follow her footsteps to work in the healthcare field. My mom is my biggest mentor as she continues to teach me how to take care of others in her 80s.
I love to work with elders as it’s deeply meaningful to me. The PACE approach towards practicing patient care at CEI centers works well as I can take the time to truly listen, have empathy, foster trust and build connections with our participants. Equally meaningful to me are the relationships I have with my colleagues. They inspire me each day through their dedication to our participants. Their compassion, intelligence, and deep expertise in elder care continually motivate me to grow as a clinician.
I started my nursing career at Center for Elders’ Independence in 2018 as a medical clerk at the San Leandro Clinic. Soon, I was promoted to lead Patient Care Navigator for the Home & Community Based Alternative (HCBA) Waiver program that CEI manages for Alameda and Contra Costa Counties. After getting my RN license, I worked as an RN Case Manager for the CEI HCBA program. Seeing that I could truly make a difference in the lives of seniors, I continued my higher education and received advanced practice provider training at UC San Francisco, becoming a nurse practitioner in Gerontology. I’m thrilled to achieve this milestone and serve CEI seniors with PACE medical care.
I speak English and basic Korean.
Outside of CEI, I really enjoy foggy morning walks with my chocolate labrador retriever, Juno, and volunteering at the local SPCA with my daughter on the weekends. I also love to go on beach vacations with my family and exercise at new CrossFit gyms whenever we travel. I am a San Francisco native and have lived in the SF Bay Area most of my life.
