The gratitude the participants express gives me the strength to come to work every day. They enrich me with their history, backgrounds and cultures and is my favorite part about working with elders. PACE provides me with a unique environment to be creative with problem-solving skills to address the complex issues and barriers that seniors face on a regular basis.
I was raised on a Marine military base in San Diego and moved to Northern California in my mid-20s after studying abroad. I earned my Registered Dietitian’s license in 2020, a month after joining Center for Elders’ Independence.
After PACE Concord opened, I stepped into a leadership role becoming the Day Center Supervisor. Now at Tri-Valley, I am thrilled to be the Center Manager.
I speak English and very limited Spanish.
For my time outside of work, my fiancé and I enjoy cooking all kinds of foods as we are both dietitians and big-time foodies. We also stay active by hiking the many trails here in the Bay Area and have fun with our two cats, Tonks and Diggory.

Every day, I am inspired to work with seniors as they value their independence as much as I value theirs. I see them continue to enjoy living life to the fullest. I am passionate about the way PACE puts a lot of emphasis on how each team works together to make a difference in the seniors’ lives and to keep them safe at home as long as possible.
As a medical student at the University of Santo Tomas located in the Philippines, I earned my medical degree placing myself on the Dean’s List and graduating with high distinction (cum laude). After completing my studies, I did my geriatric medicine with palliative care training at Mount Sinai St Luke’s Roosevelt Hospital in New York City.
My dedication in working with frail elderly patients has been recognized by the MidAtlantic Medical Directors Association and I was awarded the Outstanding Clinical Physician of the Year 2018.
Currently, I am a Diplomate for the American Board of Internal Medicine and am the primary care physician at Center for Elders’ Independence, San Leandro Center joining in 2019. I’m also an active member of the American Geriatrics Society and American College of Physicians among others.
I recently earned my Board Certification in Geriatric Medicine.
I speak English and Filipino fluently as well as medical Spanish and basic Chinese.
When I have time off, I find myself enjoying nature trips and discovering new places worth exploring. I also like to stay active by going to the gym regularly and spending quality time with my family and friends, including our dog at home.
Albertonia “Toni” Woods Johnson
Albertonia “Toni” Woods Johnson
Albertonia “Toni” Woods Johnson joined CEI in November 2025 and brings over 27 years of nursing experience to her role as a PACE primary medical provider. Originally from Birmingham, Alabama, Toni earned her undergraduate degree in nursing from Jacksonville State University in 2012, followed by a master’s degree in nursing in 2015 and a second master’s degree as a Family Nurse Practitioner in 2018. She is a two-time inductee into the Sigma Theta Tau Nursing Honor Society and is nationally board certified through the American Association of Nurse Practitioners (AANP) since 2018.
What draws Toni to PACE is its holistic, team-based approach to care that empowers elders to remain healthy, independent, and engaged in their communities. She is inspired by the program’s mission to improve quality of life and values the opportunity to build meaningful relationships with participants and their families.
Working with elders inspires Toni because of the wisdom, resilience, and life stories they share. Her passion for geriatrics began during her time in Geriatric Rehabilitation as an RN, where she witnessed firsthand the impact of compassionate care on recovery and dignity. That experience sparked a lifelong commitment to supporting older adults in living their best lives.
Outside of work, Toni enjoys traveling throughout California with her husband, exploring new destinations and hidden gems. She loves experimenting with recipes from around the world and nurturing her collection of houseplants—hobbies that bring creativity and relaxation into her life. Toni and her family have lived in the Bay Area since 2019.
