Our team of senior care experts get you the medical care that you need. All of our providers specialize in providing care tailored to the unique health needs of seniors. Each of our PACE locations have an associated medical clinic so that you can easily meet with your doctor.

Care Coordination

Assisting caregivers and seniors who want to maintain their agency while navigating health care.

Our care coordination service provides you with your own care team that understands your needs. Your care team will work with you and your doctors to create a personalized care plan to help you thrive while maintaining your independence.

We manage all the logistics for you – the paperwork, prescription refills, transportation to and from medical appointments, and communication across your entire care team. We also manage and maintain your paperwork with Medicare and Medi-Cal.

Home Care

Support in the comfort of home for independent seniors that need help with daily tasks.

Our care services are not limited to our PACE locations. If you need assistance at home with light housekeeping, bathing, dressing, and other daily tasks, our compassionate caregivers can assist you as needed, ensuring comfort and dignity in every step.

Comprehensive Care at Little to No Cost

We’re here to make healthcare accessible and affordable. Most participants qualify for full and partial coverage of our programs.

Medi-Cal Only

$0

If you are currently enrolled in Medi-Cal, there are no co-pays for PACE services. If you have share of cost, you must continue to pay your share of cost to CEI.

Medi-Cal & Medicare (Dual Coverage)

$0

If you are currently enrolled in Medi-Cal and Medicare, there are no co-pays for PACE services. If you have share of cost, you must continue to pay your share of cost to CEI.

Medicare Only

Monthly Premium

If you are currently enrolled in Medicare, you will pay a monthly premium for PACE services.

Medi-Cal Only

$0

If you are eligible for Medi-Cal and meet the care requirements for the HCBA Waiver Program, then the program is fully covered.

Medi-Cal & Medicare (Dual Coverage)

$0

If you are currently enrolled in both Medi-Cal and Medicare, then the HCBA Waiver Program is fully covered by Medi-Cal.

Private Insurance

Not Covered

The HCBA Waiver Program requires full Medi-Cal eligibility and is not a program that is covered by private insurance.
